Policy Number (for example, ART 1234567 89)
ART BOP CFP CPP GLP HOP LLP RLP ULC Required Check the Policy Number. Check the last two digits of the Policy Number.

Mailing Address Zip Code (for example, 12345)
Required Zip code consists of 5 digits.

Online Access Code (for example, ABCD12345E)
Required Verify the Online Access Code.
